Responsibilities
- Provide RN-led health plan member care management across inpatient and outpatient settings, focusing on whole-person care.
- Assess member needs, develop and implement comprehensive, individualized care plans, and monitor outcomes.
- Coordinate services across care settings, facilitate referrals, and promote continuity of care to support cost-effective management.
- Conduct utilization review, advocate for appropriate resource use, and contribute to discharge planning and transition of care.
- Collaborate with interdisciplinary teams, providers, members, and families to align care with member goals and program guidelines.
- Document assessments, care plans, interventions, and outcomes accurately in the member record; communicate changes to the care team.

Requirements
- Active RN license in California
- Education: BSN preferred; ADN with relevant experience considered
- Certification: CCM or related case management certification preferred
- Experience: 1+ year of RN experience in case management, care coordination, or utilization review
- Skills: Strong communication, critical thinking, and organizational abilities; ability to work in a health plan care management setting; proficient documentation and care coordination
